Sobrante, CA | $31.00 – $46.50/hour Schedule : Full-Time |
Monday–Friday, 8am–5 pm Department: Geriatric Rehabilitation | PACE
Program Are you a compassionate Physical Therapist Assistant who

workers, and rehabilitation professionals to support seniors in
maintaining mobility, dignity, and quality of life. The Role Under
the supervision of a Physical Therapist, you will: Implement
individualized treatment plans focused on safety, independence, and
functional mobility Provide therapeutic interventions including
exercise, gait training, neuromuscular re-education, manual
techniques, and physical modalities Educate participants and
caregivers on assistive devices, mobility strategies, and adaptive
equipment Lead and assist with group therapy sessions Collaborate
with an interdisciplinary care team to ensure coordinated,
high-quality care Document services accurately and maintain therapy
equipment and supplies Travel locally to participant homes and
multiple program sites as needed Qualifications Associate’s degree
from an accredited Physical Therapist Assistant program Current
California PTA license (required) CPR and First Aid certification
Valid California driver’s license, current vehicle insurance, and
reliable transportation Experience with frail elderly or geriatric
populations preferred Strong communication skills and collaborative
