Vivo HealthStaff is seeking a Nurse Practitioner in Fremont, California for a permanent full-time position.


Job Description:
The Nurse Practitioner will work with a large non-profit organization providing healthcare services to the geriatric population of the Bay Area.
Details:

• 
  
• 40 hours a week
  
• Monday to Friday (8 am-4:30 pm)
  
• Patient volume: 5-6 patients per shift in an out-patient clinic
  
  
  
  DUTIES/RESPONSIBILITIES:
  
  
• Participates in the health assessment and examination of patients.
  
• Evaluates the needs and participates in the development of a comprehensive program and nursing treatment for each patient.
  
• Manages minor illnesses, injuries, and the stable, chronic illnesses of patients according to standards of procedures.
  
• Evaluates the acute illnesses of the participants.
  
• Coordinates appropriate referrals to healthcare providers.
  
• Provides clinical assistance to RNs.
  
• Fulfills all administrative requirements including statistical records, chart entries, etc.
  
  
  QUALIFICATIONS:
  
  
• Active California license as a Nurse Practitioner
  
• One year of experience working with the elderly population.
  
• Experience as an NP
  
• Outpatient Clinic Epic Experience
